Who pays for occupational health cards: employer or employee?

Should the employee or the employer be responsible for the cost of the occupational health card (green card)?
Is the employee required to obtain an occupational health card if they are not yet legally employed (under a spouse's visa) and do not yet have a work permit?

The employers are responsible for shouldering the cost of an occupational health card (OHC) for their employees.
If an employee is under a spouse visa, they may be required to obtain an OHC in order to obtain a work permit.
